Attestation of Documents
Sworn Affidavits
The applicant should come in person to the Consulate General with a valid Sri Lanka passport, valid Swiss visa and original documents that are relevant to the Affidavit (Eg, Driving license, Birth certificate, Marriage Certificate).
Applicants should come with a prepared affidavit in an appropriate format along with related documents proving the content of the affidavit.
A set of copies of all documents including identification documents (passport, visa/ID) should also provide.

Where a Power of Attorney is required for submission to a Court of Law in Sri Lanka or any other legal institution or matters related to legal implications, the applicant should obtain the Power of Attorney from a lawyer in Sri Lanka and bring the document to the Consulate General along with the following documents that will be signed in the presence of an officer at the Embassy;
Power of Attorney prepared by a lawyer
Applicant's valid passport and Swiss visa
Two witnesses with valid passports Swiss visa

In case where one parent is in Switzerland and the other parent is in Sri Lanka, when a passport application to obtain a new passport for a child is submitted to the Department of Immigration and Emigration in Colombo, the parent in Switzerland should:
write a letter addressed to the Controller General/ Immigration and Emigration, Colombo stating he/she has no objection for the issuance of a passport to the child and submit the same to the Consulate General for attestation along with his/her passport and a copy of the birth certificate of the child.

Issued by the parent living in Sri Lanka
The affidavit should be issued by the mother or father (holder of Sri Lankan Passport/NIC) of the applicant (holder of Sri Lankan Passport) confirming that the applicant is unmarried. If the applicant is a divorcee or widow/widower, the affidavit should clearly mention the same. If the parent declaring the affidavit is living in Sri Lanka the affidavit should be attested by an Attorney- at- law or by a Justice of Peace in Sri Lanka.
An affidavit attested by an Attorney- at- law should be first certified by the Registrar of the Supreme Court. If attested by Justice of Peace it should be first be certified by the Ministry of Justice. Then it should be submitted to the Consular Section of the Ministry of Foreign Affairs/Colombo.
Issued by the parent living in Switzerland
The affidavit should be issued by the mother or father (holder of Sri Lankan Passport) of the applicant (holder of Sri Lankan Passport) confirming that the applicant is unmarried. The mother or father should come in person to the Consulate General to issue the affidavit.
If both parents of the applicant have passed away, the affidavit should be issued by his/her sibling. In such case the original or certified copy of the birth certificate of the sibling should be submitted to the Consulate General and the sibling should come in person to the Consulate.
If both parents of the applicant have passed away, original or certified copy of the death certificates should be submitted to the Consulate General.
If the applicant is a divorcee or widow/widower, the affidavit should clearly mention the same. and submit the original or certified copy of the death certificate of the spouse or the original or certified copy of the divorce certificate as applicable.
